READ CAREFULLY – THIS IS A LEGAL DOCUMENT

By signing this document, you agree to release and discharge 8 Sins Muay Thai, its owners, employees, coaches, volunteers, and affiliated parties (collectively “Released Parties”) from any and all liability for injuries or damages resulting from participation in any activities at 8 Sins Muay Thai.

1. Assumption of Risk

I understand that Muay Thai and other martial arts or fitness activities carry significant risks, including but not limited to: physical contact, sprains, broken bones, concussions, dehydration, and other serious injuries. I voluntarily assume all risks associated with my participation.

2. Medical Fitness

I certify that I am physically fit to participate and have no medical condition that would prevent me from safely engaging in strenuous physical activity. I consent to receive emergency medical treatment if necessary.

3. Release of Liability

In consideration of being allowed to participate, I hereby release, waive, and discharge the Released Parties from all liability, claims, demands, actions, or causes of action related to any loss, damage, injury, or death that may be sustained while participating in activities or using facilities, whether caused by negligence or otherwise.

4. Rules and Behavior

I agree to follow all rules and instructions provided by 8 Sins Muay Thai staff. I understand that failure to do so may result in suspension or termination of participation without refund.

5. Media Release (Optional)

I grant permission for photos or videos of me taken at 8 Sins Muay Thai to be used for promotional or marketing purposes.

6. Governing Law

This waiver is governed by the laws of the state of California. If any part of this agreement is held unenforceable, the remainder shall remain in effect.
